The WVU School of Public Health Coordinated Entry and Assessment Program
Are you an individual looking for assistance or are you part of an agency looking to learn more about our programs?
The Office of Coordinating Health Services provides an initial point of contact for individuals who are homeless, who are at an increased risk of homelessness, and/or who experience a disabling condition. The goal of the program is to eliminate barriers to essential housing services across West Virginia. We collect information and necessary documentation from the client and we link them to local service providers. Client needs can be assessed and they can be matched to programs that provide services in their area.
Coordinated Entry and Assessment also works with local service providers across the state to collect documentation and make referrals to housing programs. Coordinating these services reduces the assessment and data collection time. Our office aims to efficiently capture relevant information, collect necessary documentation, share information with providers across the state, and reduce the obstacles faced by individuals who need services.
